It's one of the great mysteries of genderswap, along with Hair Magically Grows To Feminine Lengths! and Skirts Make All the Difference, Really! and stuff like that.

But, for a slightly less snarky answer, I think that it's because menstruation is one of those things that is exclusively female and which has no male equivalent, including the hormonal changes and the cramps and whatnot, so when it's made extreme like that -- agonizing cramps, crazy mood swings, etc -- it's kind of like the author saying, "OH YEAH, HAVE SOME MENSTRUATION, WIMPY BOY, THIS IS WHAT WOMEN DEAL WITH ALL THE TIME!" without actually dealing with the part where a lot of women...don't.

But honestly, I think that male to female genderswap is one of those things where femaleness gets exaggerated to a kind of comical degree. or something.
